Can any hero use any weapon or armor? For instance, could the scout Jain Fairwood use a Rune weapon, or one of the mages use a huge melee weapon?

As has been mentioned, there are no archetype or class restrictions on either weapons or armor.
However, note that armor as heavy as Chainmail has two restrictions: 1) you cannot equip Runes, and 2) Your speed is reduced to 4 and cannot be increased. The Plate mail knocks you down to 3.
So while there are no built in restrictions, there are some limitations to combination of gear, and I doubt the scouts want their movement restricted.

Plus, many class-specific skills can only be used in combination with weapons of a certain type.

On the subject of weapons, has the Off-Hand bonus ability been ditched? I really liked the idea of dual wielding.
There is no off-hand bonus. But, you can dual wield. It's just you have to choose one weapon or the other when you attack. This gives you an advantage of having more options, but that's it.

One of the classes (Berserker, I think) has a class skill about dual-wielding. The concept is not entirely gone, it has just been redesigned.
