Has anyone played the multiplayer scenario yet? If so who are the commanders/houses that play and how did it go?

Not played yet, but the commanders in scenario 33 are:
Rickard, Eddard, Greatjon, Brynden, Rodrik, Robb, Marq, Edmure
vs
Kevan, Tyrion, Tywin, Timett, Shagga, Chella, Addam, Sandor

Are there any substitution rules for the players who don't own all the other expansions?

No.
Scenarios 27, 28, 29 (and 34 A) are playable with only core set and Baratheon.
The others need additional expansions.
