Page 8-9 is the basic, non-Lore set-up.
0. Select the scenario
1. Place the board
2. Place terrain
3. Place supplimental markers/terrain - the landmarks in this step are ONLY the ones called out in the scenario set-up "specially denoted"
4. Place figures
5. Pass out summary cards
6. Shuffle & Deal Command cards.
7. Read the scenario notes / set unused Command cards next to the board
8. First player starts the game.
The rules are modified as such when Lore masters are used (pp. 47-48. This is assumed since it is not clear:
4.5. Place War Council (I am using the number 4.5 to be consistent with CtA Organized)
6.1. Shuffle & deal Lore cards
6.2. Take Lore Token(s)
7.1. Shuffle discarded Lore cards into unused Lore cards and set next to the board
Page 58 and 65 seem to add the additional steps (Again assumed numbering):
0.1. Determine the number of Lore Council Levels to be used
7.9. Place Landmarks for Level 3 Lore Masters
Epic adds that card are to be placed in the Epic Rack before the 1st players turn. Since all other cards are out at this point, I assume this should also happen before Landmarks are placed, so I will call this step 7.2.
(everything in italics is an assumed step) I am placing "Place Landmarks" as step 7.9 because it is to happen at the start of the game. The start of the game, based on p. 9, is step 8.

The only objection I would have would be with step 7.9 - it would seem more natural to have near the Call Specialist phase, since there could be some placement of items at that stage as well. I do agree that it should be probably after that phase, should there be a disagreement for being before or after the Call Specialist phase. And like I said, just seems "natural" to have it at that point to place any additional tiles then. Obviously I know there are some scenarios where you aren't suppose to place the Level 3 terrain and they would be in the scenario notes.
Also, maybe I'm not seeing it but I don't see a place for placing the creature terrain.

I suppose it is open to interpretation, but seems clear to me that Landmarks are intended to be placed after all command and lore cards are drawn. "Before the game's first turn" does leave some ambiguity, but I think it's reasonable to interpret that as the last action done before the game begins.
Creatures' Lairs and Lore Masters' Landmarks are talked about the same way in the rulebook. The way we've been playing it is that the player going first places all landmarks, be the Lore Master or Creature Lair, then the player going second places theirs.
Half the world starves,
Half the world bloats,
Half the world sits on the other half and gloats
Creature Lairs are Landmarks. Or, at least they are treated that way in the rules.
I placed "7.9" as the very last thing not based on my preference but based on the reading of the rules. I would think the placement of them would lend itself to some strategical thinking and it is best to do that with as much info as possible. The last bit of info you have before playing is getting you cards.
